"Eating the apple" is a reference to one of the changes that masturbation makes to the human body. The bible's admonishment to "Don't eat the apple", is an admonishment not to masturbate.

The face made by the Japanese created cartoon character in the picture above is an insult. According to Happeh Theory, the insult is meant to indicate the person's body has been changed by masturbation, or one of the other actions that cause similar changes to the body that masturbation does. Excessive Exercise, Excessive Sex, Homosexuality, Injection of Drugs, Snorting of Drugs, and Anorexia.
This insult can also be used to indicate that a person is unbalanced, not mentally all there, or slow and stupid. Those meanings would be more associated with a person whose body has been changed at birth in the same way that masturbation changes the human body, than a person whose body was changed by masturbation or one of the other listed causes.
The insult in those cases would be indicating the person was a lower class person who could never be anything but what they are because of the changes to their body.
The insult could also be interpreted as meaning a person had not grown up properly or evolved into a real human being. A human body might exhibit the same changes that masturbation causes to the human body, but only because that human body has not matured or grown up properly.
The affected individual could change their body so that it no longer showed the changes associated with masturbation. The fact that they had not done this is the basis of the insult. The insult would mean something like "why are you grown up in years but your body is still the body of an immature child?".
